● Form filling instructions

① Form collection: At the reception desk of the campus hospital in the registration hall of the Chengxin Building.

② All forms and certificates must be filled out clearly and neatly, and no alteration is allowed.

③ A one-inch photo is attached to the health examination form, and the students must fill in all the columns clearly. If you are not suffering from an acute or serious illness, please fill in “none”.

● Stool quarantine

① According to the notice from the Beijing Center for Disease Control and Prevention, all new students entering the w88 casino must undergo stool quarantine. Students who do not participate in quarantine will not be registered. The w88 casino hospital will issue stool specimen boxes to new students when they register.

② Submission time: the day of physical examination7:00－15:00。
