●Instructions for filling out the form

① Form collection: At the reception desk of the campus hospital in the registration hall of the Chengxin Building.

② All forms and certificates must be filled out clearly and neatly, and no alteration is allowed.

③ A one-inch photo is attached to the health examination form, and the students must fill in all the columns clearly. If you are not suffering from an acute or serious illness, please fill in “none”.

●Stool quarantine

① According to the notice from the Beijing Center for Disease Control and Prevention, all new students entering the w88 casino must undergo stool quarantine.
