Instructions for physical examination and quarantine for new students

Form filling instructions

☆Form collection:Reception desk of the campus hospital in the registration hall of Chengxin Building;

☆All forms and certificates must be filled out clearly and neatly, and no alteration is allowed;

☆A one-inch photo is attached to the health examination form, and the students themselves must fill in all the columns. The past column must be filled in truthfully so that the w88 casino doctor can provide health guidance to the students during their time at w88 casino. If you are not suffering from an acute or serious illness, please fill in “none”.

☆Fill in your name, gender, and age on the inspection form, and fill in the name of the w88 casino and department in the upper left corner of the form.

Physical examination arrangement

☆Physical examination location: In the campus hospital on the first floor of Zhixing Building;

☆Physical examination time: see the table below;

☆Physical examination requirements: Bring physical examination form, laboratory test sheet, fasting, on the morning of the designated examination day6:30-10:30Blood test,7:00-12:00Physical examination,After the examination, the form must be returned to the Health Department of the campus hospital for archiving.

Stool quarantine

According to the notice from the Beijing Center for Disease Control and Prevention, all new students entering the w88 casino must undergo stool quarantine. Students who do not participate in quarantine will not be registered. The w88 casino hospital will issue stool specimen boxes to new students when they register.

☆Examination time: the day of physical examination7:00－15:00

☆Submission location: Outside the outpatient hall of the w88 casino hospital, in the stool specimen storage box.

☆request:First write the college, department and name on the tape of the stool specimen box:Use a bamboo skewer to pick out fresh stool about the size of soybeans and put it into a box and send it to the w88 casino hospital;Keep the outside of the specimen box clean.

Immunization arrangements

Wait for the free vaccine after enrollment and wait for notification from the w88 casino hospital for unified vaccination; for self-funded vaccines, fill in the vaccination registration form when registering for voluntary vaccination(Specific matters will be arranged by the w88 casino hospital when registering)。
